	Complete the Claim Form in as much detail as possible.
	Post or fax your Claim Form to us as soon as you can.

Important: If your goods are stolen, please immediately report the theft to the police. We will need a police case number before we can process your claim.

All documentation needed to process your claim must be sent to HomeChoice within 60 days of the claim date.
